If you’d like to make changes to your child’s current permanent bookings, please complete the Change to Permanent Bookings form below.
All additional bookings are subject to availability and are only confirmed once approved by both the service and Administration.

If you wish to cease care, please complete the Cancelling Care form below and return it to enrolment@adventurepatch.org.au

You must give at least two (2) weeks’ notice when cancelling care.
Please note: Child Care Subsidy (CCS) is not paid for absences after your child’s last physical attendance, so your child must attend on their final day of care.
If they do not attend, CCS cannot be applied to those absences, and full fees will apply in accordance with Government regulations.
